Liability Wiaver

All GreenHorn Bass Tour Participants must agree to the following agreement to participate in GreenHorn Bass Tour Events and Activities.

Release of Liability and Assumption of Risk Agreement

In consideration of being allowed to participate in any way in the Greenhorn Bass Tour (the “Tour”), its related events and activites, I,

the undersigned (the “participant”) acknowledge, appreciate, and agree that:

Acknowledgement and Assumption of Risk of Injury. Participant understands and agrees that there are risks of significant injury to the Participant or any invitee of Participant during the activities involved in Tour. Participant understands and agrees that these risks of injury include accidents or incidents which may result in injury, harm or other damage to persons or property and Participant KNOWINGLY AND FREELY ACCEPTS AND SPECIFICALLY ASSUMES ALL SUCH RISKS, both known and unknown. Participant shall be solely and fully responsible for all acts of Participant or any invitee of Participant.

Waiver of Liability and Indemnification Agreement. Participant agrees on his/her behalf, and on behalf of Participant's invitees, to release, defend and hold the Tour and its directors, agents or representatives (“Tour Personnel”), other participants, sponsoring agencies, harmless from any and all liability, claims, demands, actions, proceedings, costs (including attorney fees) and expenses of any kind including, but not limited to, those arising from bodily injury or death to any person or loss of value or damage to any personal property sustained by Participant while engaged in the activities of the Tour.

Participant Responsibility for Decisions While Engaged in Tour Activities. Participant willingly agrees to comply with the stated rules (including the requirement to procure and maintain adequate insurance coverage) and the customary terms and conditions for participation in the Tour. If, however, Participant observes any unusual significant hazard during participation in the Tour activities, Participate will cease his/her participation and bring such matter to the attention of the Tour Personnel immediately. In the event that Participant or an invitee of Participant is involved in an accident or incident during its participation in the tour, he/she shall immediately notify the Tour Personnel. Additionally, should Participant or any invitee of Participant require medical attention while participating in the Tour, the Participant shall be solely responsible for making all decisions related to medical care and survival procedures for the Participant or any such invitee of Participant, including, but not limited to, all decisions to seek or reject medical care, administer medical care while onsite at the Tour location, perform life-sustaining procedures, and/or transportation and admission to any hospital or medical clinic.

I HAVE READ THIS RELEASE OF LIABILITY AND ASSUMPTION OF RISK AGREEMENT, FULLY

UNDERSTAND ITS TERMS, UNDERSTAND THAT I HAVE GIVEN UP SUBSTANTIAL RIGHTS BY

SIGNING IT, AND SIGN IT FREELY AND VOLUNTARILY, WITHOUT ANY INDUCEMENT.
